The new CRM clinic model targets adult patients (aged 20–80) identified as having high or intermediate risk for cardiovascular disease (CVD), chronic kidney disease (CKD), diabetes, or obesity, using a bespoke EMIS search logic.
To understand whether the new clinic approach is working as intended and how it can be improved. The evaluation will help us answer questions such as: · Are patients receiving better care and support for their heart, kidney, and metabolic health? · Is the new way of delivering appointments making a real difference to people’s health and wellbeing? · Are there any groups of patients who are not benefiting as much, and how can we address this? · Is the service providing good value for money for the NHS? The findings from this evaluation will be used to make decisions about how care is provided in the future. This means that patients will benefit from services that are based on evidence, tailored to their needs, and designed to achieve the best possible health outcomes for everyone.
